A study of more than 20,000 adults links very high levels of an inherited blood particle to a greater risk of stroke and cardiovascular death.
A large new analysis has drawn fresh attention to a common but often overlooked cardiovascular risk factor, one that around one in five people carries without knowing it. The particle in question is lipoprotein(a), usually written as Lp(a), and researchers say very high levels of it are independently linked to a greater risk of major cardiovascular events, particularly stroke and death from cardiovascular disease.
The findings come from an analysis of more than 20,000 adults and were presented as late-breaking research at the Society for Cardiovascular Angiography and Interventions 2026 Scientific Sessions. Because elevated Lp(a) rarely causes any symptoms, most people who have it remain unaware, which is part of what makes the results significant for public health.
What Lipoprotein(a) Is and Why It Matters
Lp(a) is a cholesterol-carrying particle found in the blood. It resembles LDL, the so-called bad cholesterol, but carries an additional protein that may increase its harmful effects on the arteries. Its levels are mainly inherited, and they can raise cardiovascular risk even when a person’s routine cholesterol readings appear normal.
This is what sets Lp(a) apart from the cholesterol measures most people are familiar with. Standard lifestyle changes such as diet and exercise, which can meaningfully lower LDL cholesterol, have limited effect on Lp(a), because levels are largely determined by genetics rather than habits. Its connection to heart disease has been recognised for years, but its usefulness in predicting risk across different groups of patients has been less clear, which is what this analysis set out to examine.
The researchers analysed stored blood samples from more than 20,000 adults aged 40 and older who had taken part in three major clinical trials run by the US National Institutes of Health. Participants had an average age of around 65, and roughly 65 per cent were male. They were grouped according to their Lp(a) levels and according to whether they already had heart disease.
Over a median follow-up of just under four years, about 7 per cent of participants experienced a major adverse cardiovascular event, a category that includes heart attack, stroke, an artery-opening procedure, or cardiac death. People with the highest Lp(a) levels were about 30 per cent more likely to suffer a major cardiovascular event, nearly 50 per cent more likely to die from cardiovascular disease, and about 65 per cent more likely to have a stroke, compared with those who had lower levels.
This elevated risk persisted even among patients who were already receiving standard treatment, which the researchers say suggests people with high Lp(a) may require more aggressive management of their other cardiovascular risk factors.
Why a Simple Blood Test Could Make a Difference
One of the study’s central messages is that this hidden risk can be identified easily. Elevated Lp(a) can be detected with a simple, low-cost blood test, yet it is not routinely included in the standard cholesterol panels most people receive, which is a large part of why so many cases go unnoticed.
The researchers stressed that knowing one’s status opens the door to action. If elevated Lp(a) is detected, they advised that patients work closely with their healthcare provider to lower LDL cholesterol and manage other cardiovascular risk factors as far as possible. They also noted that this knowledge is becoming more valuable as new targeted treatments move through late-stage clinical trials, raising the prospect that Lp(a) itself may become directly treatable in the coming years.
The team plans to extend its work to additional patient groups in future analyses, including people with chronic kidney disease and peripheral artery disease. The researchers say the findings reinforce a growing view that measuring Lp(a) could become a more routine part of assessing cardiovascular risk, especially for those who keep their LDL cholesterol under control but remain at risk for reasons that standard tests do not capture.
